NOTICE TO CREDITORS
(Gerald E. Maddox and Carol M. Maddox and the Maddox Family Trust)
Notice is hereby given that the undersigned is the duly appointed and qualified trustee the Maddox Family Trust dated March 18, 2006, and any subtrusts therein. On September 27, 2012, Gerald E. Maddox died, and on March 19, 2022, Carol M. Maddox died. All creditors having claims against Gerald E. Maddox or Carol M. Maddox or the Maddox Family Trust dated March 18, 2006, must file claim with the undersigned at the address given below within 90 days after the mailing or the date of first publication (as the case may be) of this notice.
